Students must be registered each sports season via the above link

This must be completed for each sports season, regardless of prior season participation
The registration portal shall be open approximately 30-60 days prior to the start of the season

The NJSIAA physical form must be submitted during the registration process.
This form remains valid for 365 days following the date of the exam.

Students may not participate if their physical is expired or not submitted on the NJSIAA form.

All middle school students need to have a physical if they are going to participate in school-sponsored sports. The physical forms linked below are the ONLY form accepted for participation in sports. We may not accept alternate forms. Physicals remain valid for 365 days and must be approved by the school physician.

A copy of the physical MUST be submitted to the health office to complete registration. This may either be a hard copy or scanned copy emailed to healthoffice@wbps.org

You are strongly encouraged to retain a copy of this physical for your own records.
