Physicals

All middle school students need to have a physical if they are going to participate in school-sponsored sports.  The physical forms linked below are the ONLY form accepted for participation in sports.  We may not accept alternate forms. Physicals remain valid for 365 days and must be approved by the school physician. 

A copy of the physical MUST be submitted to the health office to complete registration.  This may either be a hard copy or scanned copy emailed to healthoffice@wbps.org

You are strongly encouraged to retain a copy of this physical for your own records.

Elementary school students needing physicals can utilize this form; Middle school students that have no intention of participating in sports may also utilize this form
