Medical registration

Parents wishing to register their child for enrollment into WBPS must provide the following medical documentation in addition to the other registration documents (birth certificate, proof of residence, etc):

• Immunization record - Students without a complete vaccine record will not be admitted to school

  • NJ DOH vaccines Pre K

 

• Recent physical examination -Students not presenting a valid physical in a timely fashion may be subject to exclusion

  •  Physicals

 

• Students entering school from outside the US may be subject to prove that a tuberculosis test was performed since immigrating.
